Syntax of this template is incorrect. Currently reads:

Ag baint "$1" amach, scrios $2 é ón Cómhaoin mar: $3.

Should be something like:

Ag baint [[Íomhá:$1]] amach, agus ag chur [[Íomhá:$2]] isteach mar ionadúchán. (Scrios [[commons:User:$3|$3]] é ón Cómhaoin mar: $4)

Equivalent to:

Replacing [[Image:$1]] with [[Image:$2]] (by [[commons:User:$3|$3]] because: $4).

Sounds good, except maybe:

Ag baint [[:Íomhá:$1]] amach, agus ag chur [[:Íomhá:$2]] isteach mar ionadúchán. (Scrios [[commons:User:$3|$3]] é ón Cómhaoin mar: $4)

RE: ":Íomhá" v "Íomhá". Yup. Agreed. (Apologies - Forgot about the ":" prefix to force the link).

RE: "summary-I18n" Úsáideoir:CommonsDelinker/summary-I18n is there already, and is working fine. (It accepts 3 params and is handling them correctly today).
